Wiki Billing an Assisted Living Facility

Our provider frequently sees skilled nursing home patients in our office, but recently he saw an assisted living patient in our office who only has Medicare part A (does not cover our professional office visit charge). Do I bill the assisted living facility who brought her here or do I bill the patient directly? Thanks.
