Wiki billing an em code with a 11200

My doctor saw a long term pt in the nursing home. She had a horney type mole/skin tag that he did a suture ligation on. The question is should we bill a 11200 instead of the nursing home procedure 99308 or bill with a modifier?

What was the reason for the visit?
 
The doctor went in for a follow up to conjunctivitis and the patient had itchy dry skin. The nurse while he was there had him check the mole.
 
Add a -25 to the e/m. This way you get paid for both services. If the Dr. only performed the skin tag removal you would not bill an e/m.
