Hi,

I would like to know how to bill Bupivacaine and lidocaine in this scenario:

62282
J1030x2 - Depo-Medrol
J1100x4 - Dexamethasone
J3490x2 - bupivacaine or lidocaine

claims getting rejected from Hudson health, can anyone suggest whats going wrong with this????
