Billing Chemo Infusions with MD not Present

I have a question regarding billing for chemo infusions when a MD is not present. So, there is a PA or a NP present when the infusions are administered but there is no MD on site.

Is it legal for this to happen in the state of Arizona?
 
You must use the NPI of the provider at is on site. So use the PA or NP NPI in field 24J. You cannot use the MD number if they are not within the office suite area at the time the service is rendered.
