Wiki Billing CPT 17110 with 238.2 denial

Doctor did a destruction of a neoplasm of uncertain behavior 17110 with 238.2 and was denied by Aetna.
Note:
Neoplasm of Uncertain Behavior
Pink scaly papule located on the middle sternum.
Ddx includes: Lichen Planus-Like Keratosis vs. Irritated Seborrheic Keratosis vs. superficial BCC. This one lesion was treated with liquid nitrogen. If not resolved in 2 weeks consider a biopsy.
Any other ideas to get this paid? Change CPT vs Dx code
 
If there is no path report you cannot use 238.2 and it is not a diagnosis that can be rendered without path. It is just a skin lesion, 709.9 and is probably not going to be paid except on appeal.
