Billing facet block injections to medicare

Please help, we are not getting paid for any of our blocks done on medicare part b patients, we have read the lcd and are coding the way we thought we were supposed to , but all of our claims are denying for lcd...... Help, does anyone know how to bill these 4 days of charges

What were the injections sites, how did you code them on the claim, and what dx did you attach to the procedure codes?
 
Check your local Medicare LCD's for the supporting diagnosis codes. For cervical facets (64490-64493) we generally use 721.0 and lumbar facets (64493-64495) we generally use 721.3. Also you will want to append one of the pain diagnosis codes 338.xx
