Billing for End of Life Discussion with an office visit

We have billed in the past for an office visit of 99215-25(modifier) and 99497-59; 99498-59. Response on the EOL codes was "The procedure code is inconsistent with the modifier used or a required modifier is missing". We are IM office so, generally, we only use minimal modifiers to begin with. Any information would be fabulous!!
 
The modifier on 99497 and 99498 are not needed. The -25 on the e&m code is enough for the 99497, and the 99498 is an add on so it will never need a modifier. Some have said they need modifier -33 on the 99497 (indicates preventative) but I haven't needed to. Hope that helps!
