Billing for G0180, Physician certification for Medicare-covered home health services under a home health plan of care. Is the POS 11 if the majority of the plan development and review was performed in the office? What information from the Home Health Agency is required for billing and what item/box number(s) of the CMS 1500 are populated with this information? Is the date of certification or re-certification entered in box 24A? Lastly, are there any modifiers required? Thank you.

I bill G0180 with POS 11, use the date the provider signed the POC as the DOS with no ending date and if the patient died, use the Date of Death as DOS. MDCR has always paid. No modifiers. I work in an infectious disease office.
