When billing for hydration that is ordered bolus, is a rate or time span required?
 
I know the time requirements, but the question is - if the physician orders a NS IV 1000ml bolus, does the order need to state ___ml/hr, or a time frame of fluids (ex) over 1 hour?
I wouldn't expect to see a rate or time span if the order specifies 'bolus'. A bolus by definition is a large dose given all at once, so the hydration would just be run at the maximum rate until completely infused. Just code and bill the actual amount of time it took to run the 1000 ML.
