Wiki Billing for INR. Billing for 96417.

I have patients coming into the office and I am billing 36416 and 85610. We have an active CLIA number. I keep getting denials for modifers. Does this code require a modifier?

Also 96417. I have I have 3 drugs going for 6 hours, how do I bill for this?
Drug 1 -2.5 hours
Drug 2 -2.5 hours
Drug 3 -1 hour

Would I will 96413
96415 x 4
96417 x1

Or will I bill 96413 x1
96415 x5
96417x2 (2 additional drugs)

Are the denials you are receiving for the 85610?

This CPT is CLIA waived and requires a modifier -QW.

Try rebilling with that modifier. (85610-QW)
