Wiki Billing for Interrogation of LVAD

Can anyone help me with the proper documentation that is required for an interrogation of an LVAD?
I know this can be billed each day with an E/M but must show medical necesity.
Would it be possible to get an example of Medical Necessity and could you tell me where I can find specific guidelines?
