My provider is going on vacation and we would like to use a locum in his place. Does the claim go out under my providers name with a Q6 modifier or does the claim go out under the locums name ???

It will be billed under your doctor's name with the modifier. Go to the Knowledge Center of the AAPC as there are articles that deal with this specifically.
