Wiki Billing for new counselors

Can someone provide insight as to whether billing can occur for the following counselors:
Non-licensed (or not yet licensed) students of behavioral health programs, under supervision (may be used in times of access issues)
Provisionally licensed or "interns" (those with a temp license undergoing clinical hours with or without supervision)

Is this a per payor question, or are all payors processing the same way?

We have interns and the only payer that allows us to bill for services is our state Medicaid. The services must be under supervision and only those services allowed for the educational level they are working towards.
