Is it required by Medicare that when sumbmitting claims for a Pacemaker check in the office, that you list the date of implant on the claim form for reimbursement. If so can someone forward me the documentation or where I can find that information. Thanks....
 
When our office bills for pacemaker checks, we do not include the date of implant on the claim. If that is suppose to be on the claim, I would love the documation as well.
