Wiki Billing for Physician supervision of a CRNA

Hello,

I am new to anesthesia/pain management billing and I have a physician who states he medically supervised a CRNA for anesthesia CPT code 01935, procedure code 64493, 64494, 64495. I am trying to find out exactly how do I bill for his part of the anesthesia. If anyone can help me I would surly appreciate it.
 
if you're billing 01935 for diagnostic facets:

MD- add QY modifier
CRNA- QX modifier on separate claim
QZ- CRNA not supervised (alone on case)
AA- provider not working w/ a CRNA (alone on claim)

So, when you are billing, create one charge ticket for the MD with that QY modifier and then create a separate charge ticket for the CRNA if there's one on a case.
