Can we bill for a test dose? For example, we give an IV push of a drug to see if the patient will get an reaction? If patient does not get reaction, the drug is then infused.
 
For bleomycin we give a test dose IVP but include the time in with the total infusion time since we can't bill separate IVP and infusion for same drug

when we give carbo test dose we give SQ and use 86486
