Billing G0444 with other same day services to Medicare

Hi all,

I'm having trouble getting reimbursement for a combination of codes.

Does anyone have any experience with receiving reimbursement from Medicare for the following code combinations performed in the same day and if so, how are you using modifiers to do so successfully?
