Wiki billing hospital admissions and echocardiograms

Excellus BC/BS of CNY is stating echocardiograms (93306 and 93351) and hospital admissions (99221 - 99223) are inclusive. A 25 modifier was on the admission codes (99221 - 99223) and they still will not reconsider and pay the echocardiograms. Is there an article to send with an appeal or what else can I do. All other insurances are paying for both.
 
echo and admit same day

I called my local BC/BS and they told me to put modifier 59 on the echo, I did this and all have been paid.
