when billing 99239 are the notes stating that more then 30 minutes of time was spend or are you requiring the time to be there to bill it? My thought is if it isn't in the note then I would bill 99238 automatically? any thoughts?

Yes, in order to bill 99239, time spent for the discharge needs to be documented. Otherwise, it is 99238.
