I have a question in reference to how to bill out a LMT's services to insurance. We would like to use the code 97124. I understand that this is an insurance by insurance cause, where some pay/some don't. My question is specifically how to.
Is this service billed under the Chiropractor or Physical Therapist? Who's NPI does one use? How does this look on the claim form? Thank you for your help.
