Medicare allows it--see Claims Processing Manual, Chapter 5, 20.1. Just make sure you are using the modifiers as stated. However, we have had problems with a couple commercial carriers, but I think that was due to the way the therapists were credentialed with those carriers.
 
this should not be a problem just use the appropriate modifiers as stated, also your first listed dx code will be the V57.89 for multiple modalities see page 12 of the coding guidelines.
