Yes—65815 can be billed during the cataract post-op period if it meets the criteria for a return to the OR/procedure room and is not routine post-op care.

CPT 65815 (paracentesis of anterior chamber) is billable when:
  • There is elevated IOP requiring aqueous release
  • The intervention is medically necessary and not routine
  • It is performed in a procedure room / OR setting (not just chairside slit-lamp expression)
Encourage documentation like:

“Due to significantly elevated intraocular pressure unresponsive to medical therapy, the patient was taken to the procedure room for anterior chamber paracentesis…”
