Incident to billing: if the PA sees a patient for an office visit and the provider does a procedure on that same day, do I need to put a 25 modifier on the office visit since it is being billed under the provider even though they are two different claims?

You do not do two different claims, and yes use the 25 modifier if the conditions of significant and separately identifiable are met.
