I have been able to bill a skilled nursing admit with a hospital discharge (my Dr does everything for his patients) by adding a 25 modifier to the discharge line and changing the POS. They are suddenly getting denied and I am not finding anything about it on WPS. Does anybody know if the rules have changed?
