Wiki Billing two J3490 codes

Good afternoon. I am attempting to bill for an encounter which contains two drugs that both use J3490 - ketamine HCl and metoprolol tartrate. These two meds are being used for very distinct reasons and are NOT part of a combination drug. Therefore, I can't use KP/KQ modifiers. The main reason for the visit was for a ketamine infusion for chronic pain but the patient needed metoprolol for tachycardia/hypertension as a result of the ketamine. What modifier can I use for the metoprolol? Or, does it even need a modifier in this circumstance? Thank you in advance!

Wouldn't you assign different diagnosis codes to each drug? I would code each J code on a different line with the associated diagnosis. Not sure you'd need a modifier. Same route of administration?

I agree with what Pam said about the DX codes. If you also have their specific NDC numbers on each separate line, with a description including the name of each drug, that should make it pretty obvious as well that the 2 codes are for different items.
