Please Help! We bill under our group NPI. Where on the claim does the Group NPI have to be indicated and where on a claim does the rendering provider NPI go?
 
You don't indicate if you're asking about a CMS-1500 form or a UB-04. For the 1500 form, I use this reference from our MAC: https://www.cgsmedicare.com/pdf/5010_jobaid.pdf

The Line # references the block on the claim form with a cross reference to the electronic loop and segment.

For the CMS-1500 the rendering provider NPI goes in box J and box 31, the provider group NPI goes in box 33a with the billing address information. If your payer requires a referring provider then the referring provider's name goes in box 17 and the referring provider's NPI goes in box 17b
