Wiki Billing under Doctor when the Physician Assistant see patient

Does anyone have experience billing PA visits under a separate physician name when the physician did not see the patient during a particular visit? Is this ever appropriate?
 
INCIDENT-TO SERVICES
  • Are paid at 100 percent of the Medicare physician fee schedule.
  • Must relate to a service initially performed by the physician (cannot be a new problem).
  • Must be performed under direct supervision – when a physician (doesn't have to be same one who initially saw patient) is in the office suite/building (doesn't have to be in the room, but does have to be in the same building).
  • Cannot be billed when more than 50 percent of the visit is for counseling or care coordination.
  • May not include diagnostic testing.
  • Is ONLY applicable in an office setting
