Wiki Billing unlisted code with modifier 62/66

I have a physician that is asking if you can bill the unlisted code 30999 with modifiers 62 & 66.. Can an unlisted code be billed with these modfiiers with supporting documetation?

It appears that it may be allowed if documentation supports the CPT code. Each physician would need to dictate their own report documenting the work they specifically did for the surgery. Send in the claim on paper with the op report as it will have to be reviewed by insurance before they would pay.
