This is my first time to bill physician's visit at a custodial care facility. I was told to include the facilty address in Box 32. I included the facility address and the facility license number and my claim was rejected. Anyone who can help me?
 
It would be extremely helpful to know what the denial was for. Off the top of my head, I know you will also need the facility's NPI number in box 32 and you didn't mention that. Maybe you left that out?
