A patient presents with a bimalleolar equivalent fracture and only the lateral malleolus is reduced and then fixed with internal fixation. There was no need to do a deltoid ligament repair so a cast was applied to immobilize and allow it to heal. Does this count as a 27814 Open treatment of bimalleolar ankle fracture? If so, why? Thanks in advance.

If there is a fracture on the lateral side, but not the medial side, I would bill 27792. There are times when one side needs ORIF and the opposite side needs to be watched. That to me is more of a "bimalleolar equivalent" fracture. If there is no fracture to watch on the medial side to make sure that it is healing correctly I vote 27792.

I agree with True Blue. A bimalleolar equivalent fracture is a fracture of one of the malleoli and an injured ligament, such as a syndesmosis disruption. In which case you would bill for treatment of the malleolar fracture, and if there was repair of a ligament, you would bill for that also.
