Biopsy pathology documentation

I am trying to get some clarification on what documentation is needed when a biopsy is done in office at PCP and sent to lab. Does the provider need to add the pathology results in the office note in order to code the correct CPT and dx code? Does anyone have any information that I can send to a provider that is giving me pushback if this is needed? Thank you
