A patient comes into the office with an eye lesion and the doctor performs an excisional biopsy and removes the entire lesion and send it off to pathology. Would you code this as excision of lesion or biopsy ?

Doctor perofrmed & documented Entire Excision, it is an excision code that weighs more(major procedure) and the biopsy becomes a part of it .
