Help! We have a patient with a dx of H54.414. However, 2018 ICD-10 book states this requires a 7th digit but doesn't give any details as to how to qualify that. Has anyone else run into this?

Visual impairment - Mild or none (Score: 0): 20/60

Visual impairment - Moderate (Score: 1): 20/60 - 20/200 (this would be Category 1)

Visual impairment - Severe (Score: 2): 20/200 - 20/400 (this would be Category 2)

Blindness (Score: 3): 20/400 - 20/1200 (this would be Category 3)

Blindness (Score: 4): 20/1200 - Light perception (this would be Category 4)

Blindness (Score: 5): No light perception (this would be Category 5)
 
Does your book not give the the 7th character options? Look at all the options in the book the choice of code when you have blindness in one is what is documented as to the status of the other eye. So if you are coding H54.414 then you are saying the documentation supports category 4 blindness in the right eye, now you need to know the status of the left eye to assign you h correct code. The category you chose H54.41 indicates blindness in the right eye. Category H54.414 indicates category 4 blndness in the right eye and the 7th character A indicates normal vision in the left eye.

Look at the bottom of H54.8. There should be instructions there.
