When coding for a block along with the Anesthesia code should the diagnosis be documented on the anesthesia record as post op pain relief? Thank you in advance for any direction given!

Post Operative Pain Management

Hi, I currently use 338.18 dx for post operative pain management. Only when the surgeon requests it, it's done pre-operatively for the sole purpose of post operative pain management. Our anesthesia record states it.
Be prepared to appeal with notes.
Also for I-10 look to the guidelines.
