Blue Cross Medicare plans- bundle issue

Has anyone been getting denials for Blue Cross medicare plans for e/m codes when x-rays are done? We have just started getting denials for the e/m code (paid for the x-ray) stating it is bundled. Do these need a modifer?
 
We use em + s9088 but that is because we are an urgent care. We haven't had this issue. I hope you figure it out and let us know. I am curious and I love to keep learning. I would especially like to know in case we encounter the same issues at work late. Thanks
