Wiki BMD and modifier 25

Please advice if modifier 25 should be appended to the EM service when BMD(77080) is done on the same date. Where do I find documents related to this?

well, for Medicare and "medicare like" we add a .25 to the E/M (Part A side only)when xrays are done. But for regular commercial insurances we do not append a modifier .25 to the E/M when xray is done.
