Why is there no BMI code for adult age 40 or greater and BMI is between 30.0 and 30.9? Do we just not add a BMI code in this instance?

I am not really understanding the question. The BMI code do not go by age per se, they go by adult or pediatric. The adult has codes for a range of BMI 19 or les to 70 or greater.
