2 bone lesions are removed the right tibia with allograft reconstruction; one

proximal and one distal with 2 separate incisions.

1) Can I bill 27645 x 2?

2) Can I charge for the allograft reconstruction?

Does the opnote say "radical" or do you know that the Dx is malignant? For benign you could look at 27638.
