I have been submitting claims for this patient using codes 64612 and J0585. Now Medicare wants a modifier on J0585. On line 1 J0585, 25 units $250, Line 2 J0585, modifier JW, 75 units, no fee. They are also saying we have invalid name, strength or dosage of the drug. Not sure what Medicare wants and where to put it on the claim. I would appreciate any advice to get this claim corrected. Thank you!

I agree with putting a modifier JZ or JW on the J0585, but why would they say to list as no fee for waste? I have never done that, unless it was a free drug vial then I bill as 0.01. I also put a modifier on the 64612 with either RT/LT/50 depending on the injection. As far as invalid name/strength, etc I would check to see what is listed on the claim going out, maybe something is off in your system so it’s not displaying correct on the claim.
