Just curious, does anyone know why 64615 ( botox injection for migraine) and 64644 (botox injection extremity, in this case lower extremity) can't be coded together? Dx codes of migraine and spasticity. I know the CPT book notes these can't be coded together but was wondering why? The patient would have to make two separate appointments for treatment in order to get paid for each non-related procedure.
 
I interpret the CPT instruction to mean these two should not be coded together when reporting the same procedure. If these are two distinct procedures at two separate sites and performed at the same encounter, then it is appropriate to report them both and to assign the correct modifier (e.g. XS or 59) to unbundle them.
