Carefirst will reimburse ONLY 1 unit of 64614

My understanding botox injection will be reimbursed once per region - is an extremity one region, how do we define a region, if injection is done (even if it is theoritically possible) bilateral lower & upper extremities, how do we code?...mod LT/RT/50 Units ...?

If multiple injections are given in say one lower extremity how is it coded?
