Wiki Brain bleed versus mass

Would someone please point me to a guideline for coding when two diagnoses are listed and separated by "versus"? I've tried to find the guideline and may just be overlooking.

Thanks for replying. I'm really just looking for a guideline to use when documentation lists contrasting/comparable diagnoses, sometimes separated with the word, versus. Ex: brain bleed versus mass.

If it's not definitive you can only code the s/s.

I agree with TonyJ. At this point, you can't code it as either a brain bleed or a mass because it isn't certain. Go with signs/symptoms.
