Breast Cancer of the shoulder

Patient with history of breast cancer presents with cutaneous lesion on shoulder. Excisional biopsy performed. Pathology shows invasive lobular carcinoma consistent with her breast cancer? What CPT should i bill. I had originally sent 11606 for excision malignant lesion but denied for using breast cancer code 174.9 (non specific)? Any ideas??:confused:
 
you use a metastatic code for the cancer it is still breast cancer just located in the cutaneous area. So use a code for metastatic cancer of the skin and soft tissue and a code for the breast cancer, 174.9 if the cancer is still being treated at the breast site, or V10.4 if the breast cancer at the primary site is considered no evidence of disease with no further treatment of the primary site.
