As of Jan 1, 2019, there are new CPT codes to distinguish between unilateral and bilateral breast MRI. If a patient needs a diagnostic left MRI, should the unilateral code (77048) be billed? The radiology facility that does the MRI's is telling us that we have to order a bilateral MRI (77049) regardless because the machine has to image both breasts and the radiologist has to read everything that is imaged.
 
77048 would be correct. Magnetic resonance imaging is done on one (77048) or both breasts (77049), without and with contrast material, including real-time computer-aided lesion detection, when performed.
