breast reduction with lipo

I am at a loss here. The patient has a "nonhealing surgical wound" on the sternum and chest from previous heart surgery 8 years ago. It keeps opening up due to the weight of her breast. The doctor does a breast reduction with lipo due to the complications with healing. How would you bill this to Medicare?
