breast revision question

Hello - can anyone answer a question for me. I have a patient who has breast reconstruction after cancer. We did a scar revision under the breast for a wound separation and a large dog ear. The doctor wants to code 13101 and 13102, I think it should be 19380 or 19499. Please help
 
I query the Encoder and I'm coming up with the same codes that the doc is telling you to bill with...I think the reason is that he is actually working on a wound not reconstructing the breast...
