Informed consent was obtained for the procedure, including possible sedation. Risks of hypoxemia, lung injury/perforation, hemorrhage, infection, arrhythmia, and adverse drug reaction were discussed.

Time-Out Process performed, verified patient identification, verified procedure, verified correct patient position, special equipment available.

Prior to the administration of sedation and and neuromuscular relaxation, adequate oxygenation and ventilation was assured by adjusting mechanical ventilator to 1.0 atm of oxygen and matching the patients minute ventilation. The bronchoscope was advanced through the ETT into the trachea.

Findings: there were mild thin whitish secretion over RUL, RLL and LLL which were suctioned easily. Anatomy looks normal grossly. No airway compression. Bronchial wash sent.

Complications: None; patient tolerated the procedure well.
