If a provider is performing a transbronchial needle aspiration of a RUL cavity lesion at the RUL apical cavity lesion using a 21 guage needle for cytology, using fluoroscopy and Cone Beam CT guidance, which CPT code/codes should be used? Could anyone help me with this please? The reason I am asking is because our providers are coding this procedure with 31628 which does not say needle aspiration technique. 31629 says transbronchial needle biopsy. Hence my confusion. Any help will be greatly appreciated. Thank you.
 
31629 for the transbronchial needle aspiration and 77012,26 for the cone beam ct :)
Would you have guidance on this scenario? 77012-26 is being submitted by the provider when a live CT scan is used during Ion robotic bronchoscopy. The provider states they received this code from other facilities that are reporting it in this way. Code +31627 is also being reported for navigational bronchoscopy where a patient’ s previous CT scan is used to help facilitate biopsy. This code includes 3D reconstruction - 76376, 76377. Is it appropriate to report 77012 – 26 for the live CT scan when +31627 includes 3D rendering with interpretation and reporting of computed tomography, magnetic resonance imaging, ultrasound, or other tomographic modality with image postprocessing under concurrent supervision? There is no CCI edit between 77012 and 31627, although I’m having trouble determining if the 3D rendering is only accounting for previous CT scan and not the live CT.
 
We bill the 77012 with the 31627 if it is properly documented. 77012 is used for the needle guidance and requires a separate work station and should be documented as such, I usually look for the words "cone beam" in the documentation. I hope this helps!
