Should amplified probe codes 87491, 87591, 87511 and 87481 from a single source be billed separately or bundled under 87801?

Because they are all specified, I would bill each separately.

If they were not specified, and reported as a single result, would go with 87801.

"For each specific organism nucleic acid detection from a primary source, see 87470-87660. For detection of specific infectious agents not otherwise specified, see 87797, 87798, or 87799 1 time for each agent." (CPT 2016, pg. 559)

"For detection of multiple infectious agents not otherwise specified which report a single result, see 87800, 87801." (CPT 2016, pg. 559)
