Hey does anyone know if 74240 can be billed with 74245? I would think that the 74245 would cover everything? Am I understanding this wrong?

You are right - they build on each other.
Plain upper GI - 74240; upper GI with KUB 74241, upper GI with small bowel follow-through - 74245.
(same thing for 74246-74249)
You pick the one that covers all that you did.
